65 yrs old gentleman came with left sided hip pain with restricted movements, shortening & difficulty in walking for the past 6 months.He had history of Acetabular fracture fixation done elsewhere 8 months back. His xray showed arthritic and medialized left hip with anterior Acetabular plates in situ. Thus he planned for hip replacement surgery with keeping all inventory ready with revision cups. Intraoperatively after delivering the femur head out of socket, a huge central Acetabular defect was found while anterior and posterior walls were intact. Thus bone graft impaction done to lateralize the cup with rim fit large continuum cup used and fixed with multidirectional screws. Postoperative xrays showed well restored hip centre and limb length. We routinely performed such complex hip replacement surgeries at Sant Parmanand Hospital.
